Ashe Memorial Hospital works with local resources and community partners to connect patients with the help they need. Whether you or a family member needs transitional care, home care, nursing home placement or other supportive care, you will learn about those services before discharge.

Our care management team also helps connect patients with medical or adaptive equipment, medical supplies, personal care aides or other assistance to ensure safety and comfort at home.

Care management can also include financial help for handling costs connected to medical care or other health services.
