AMH Segraves
Segraves Care Center is a 60-bed, long-term care facility that is adjacent to Ashe Memorial Hospital. Residents are cared for by our competent and caring staff of nurses and nursing assistants. Segraves Care Center also provides physical, occupational and speech therapy for the residents. Each resident has an individualized care plan developed for them in order to provide them with their basic care needs as well as any special assistance needs.
In order to provide residents they care they need, a Family Council was formed. Both councils meet regularly to discuss topics regarding resident care. There is also an Ombudsman from the Region D Council on Aging who does an in-service with staff and family members.
Brenda Johnson, Segraves Care Center Activites Director, provides both group and individual activities for the residents and works with people in the community to bring in entertainment, such as musicians and dancers. An outdoor garden was established to provide residents who enjoy gardening an opportunity for outdoor activity.
The staff of Segraves Care Center are appreciate of the support they get for the residents, not only from friends, families,but also from volunteers from the community at large.
